We are a branch of a national, voluntary, non-profit association, funded by the Mississauga Halton Local Health Integration Network (LHIN), community United Ways, Ministry of Health Promotion: Communities in Action Fund and fundraising activities. We envision a society where every individual has a quality of life that includes health, meaningful relationships and equitable opportunities for participation in the community.

We are moving!

The primary goal for this initiative is to meet our vision for a larger and more accessible space to effectively support and meet the needs of the community. We are pleased to report that as of May 15th, we have relocated our Oakville office at Cross Avenue to our new main office – 1540 Cornwall Road, Unit 102, Oakville.

On May 26th, 2013:

· The Milton Head Office located at 388 Main Street East will be moving to – 1540 Cornwall Road, Unit 102, Oakville
· The North Halton office located at 42 Mill Street East, Acton will be moving to – 211 Guelph Street, Unit 5, Georgetown
