CMHA Halton Region Branch is committed to protecting the privacy of all individuals who receive service from us and to complying with the requirements of the Personal Health Information Protection Act.
We will collect use and disclose only that information which is necessary to provide service to you and will ask for your consent, where required by the law, before collecting information from or disclosing information to another person or organization. The law allows us to share information about you among staff of CMHA-HRB who are involved in providing service to you and with other health care providers with whom you are involved. We may also share information about you to prevent serious bodily harm to any person.
We are careful to protect information about you from unauthorized use. Information stored electronically is protected by a password and paper files are double-locked when not in use. In both instances, only authorized CMHA-HRB staff can access information about you.
If you wish to see the information we have about you, please speak to the Manager of the program where you are receiving service.
If you have any concerns about how information about you is handled, or about any part of the service you are receiving at CMHA-HRB, please speak to the staff person you are working with or to the Manager of the program where you are receiving service. We will make every effort to resolve your concerns.

The Director of Corporate Services at CMHA-HRB, is the designated Privacy Officer for CMHA-HRB. You may contact him/her if you believe your concerns or questions about any aspect of your privacy have not been dealt with to your satisfaction.
